Incident Response Action Failure Not Graceful

Description

When an incident response action fails, it causes the controlling thread to be orphaned (no longer shows up in the task list). Once it is in this state it cannot be terminated.

Incident response actions now respond better when an error occurs (this was a fairly recent change).
